That's not an issue, it's the same for everybody, that's the default location the game uses and I don't think you can change it

Maybe try something like this: Copy all the save files from the USB to your PC. Format the USB on your PS4. Save something on it from your PS4. Copy the files back from the PC to the USB. Copy from USB to PS4. That should ensure that the filesystem and folder structure on the USB is what the PS4 is expecting.

But after the new update I can't load any Fallout 4 save with it. Using the usual shortuct and Steam works fine, and I uninstalled all mods and started a new save to try again, but after I leave a save that save will always crash the game after I load, is there a fix to this, or am I just doing a dumb?
I think your saves are normally under documents/my games/fallout 4/saves. Open this in the explorer. Now open your profile folder in MO2 (you can find this under the folder symbol in the tool bar). There should be a folder called something like "MO Saves". Copy your saves into this folder, now activate profile specific save games.

Saves failing eventually, particularly quicksaves failing eventually, is a known issue. One recommended practice is to get into a safe place, start a new game after rebooting, test your save file and then delete 2 out of 3 or (if you are brave) all 3. This will give them a chance to stop going corrupt.
set your graphics options then exit. see if the Documents\My Games\Fallout4 folder was created. if not check windows security/controlled folder access. if on add exceptions for the game launcher and game. see if these files are in the folder. Fallout4Prefs.ini Fallout4Custom.ini Fallout4.ini files were created/written.
